CLS Placement Change Form

In order to better track placement requests and create a central repository for the information, CLS has created this Tier Change Request Form.  

Please note, this is ONLY for the Fall and Spring seasons.
Please use this form to make a request for a specific team to change tiers.  
Email requests, will NO LONGER be accepted

For the Fall season, this form is due to the league by July 12
For the Spring season, this form is due to the league by February 15
